From last week’s discussion, I learned that the Patient-Centered Medical Home (PCMH) model is the future of primary care focused on the patient. According to Block et al. (2020), the PCMH model implementation has the potential to reform the United States healthcare system and make it more affordable, effective, accessible, and safe for everyone. The PCMH model is comprised of seven principles; all centered around improving the quality of primary care and payments for patients. According to Ferrante et al. (2020), some principles address safety and quality improvement through evidence-based data, whole-person orientation, continuity and coordination of care, improved physicians’ access, and interprofessional team collaboration.
The principle that could be the most difficult to implement is establishing and maintaining an ongoing relationship with a primary care physician for first-contact, continuous, and comprehensive care. Ferrante et al. (2010) found that first contact is associated with positive patient outcomes, including lower healthcare costs, fewer emergency department visits, and higher patient satisfaction. The principle could be challenging because individuals move and change careers and physicians. Changing locations requires individuals to switch their primary care services, making the continuity of care with a personal physician difficult. Some services, such as mental health, could be rendered remotely, but others require the individual to be physically present, which might not be realistic if they move across the country.
My experience with a PCMH was overall positive. I saw it firsthand with my last pregnancy. Unfortunately, I suffered from postpartum depression due to childbirth complications. My primary care provider (PCP) collaborated with my obstetrician and gynecologist (OBGYN) to coordinate care with a psychiatrist to assist in my recovery. The process was smooth, but I could tell a lot was being done behind the scenes. The collaboration did not stop there. The physician trio made sure that there would be no interaction with my medicine and were mindful that I was breastfeeding. I did not have to go back and forth between the three to gather and share information because they already have all the essential information necessary to ensure safe care.
